Chloroquine and Hydroxychloroquine Toxicity PH 1200 x 630 FB Featured Image.013 Background Chloroquine and hydroxychloroquine belong to the quinolone family. They are related drugs with different therapeutic and toxic doses with similar clinical indications for use and manifestations of retinal toxicity. SD-OCT is an objective test for toxicity that complements 10-2 visual field testing. 10-2 visual testing is probably more sensitive, but is heir to interpretational difficulties and variability not so much a problem with SD-OCT. Several risk factors may increase the likelihood of retinal toxicity from Plaquenil such as, age of greater than 60 years, daily dose more than 6.5 mg/kg; use of the drug more than 5 years, obesity, preexisting retinal disease and, renal or liver failure.

Main Outcome Measures: SD-OCT findings suggestive of HCQ retinopathy before parafoveal ellipsoid disruption. Retrospective clinical data review by the Boston Image Reading Center.

Macular sensitivity changes for detection of chloroquine toxicity in asymptomatic patient Article in International Ophthalmology 302195-7 February 2009 with 10 Reads How we measure 'reads' In obvious toxicity, the nasal inner subfield showed more thinning than the temporal inner subfield at 12 months after drug cessation p = 0.018, 95 % CI −1 to −8 μm. Conclusions Once HCQ retinopathy is diagnosed and the medication is discontinued, structural retinal changes commonly occur. Although hydroxychloroquine retinal toxicity is thought to be rare, recent studies suggest it may have a higher prevalence than previously recognized. A large study examining patients with more than 5 years of HCQ use were evaluated with 10-2 visual fields or spectral-domain optical coherence tomography SD OCT.

Has anyone had hearing problems on Hydroxychloroquine. - NRAS I have had ringing in my ears when I started taking Hydroxychloroquine. I had difficulty at work coz of the constant ringing in my ears. When I mentioned it to my rheumatologist she reduced the dose. After that I dint have any probs. Its better if you try and reduce the dose and see if it makes any difference.

Plaquenil Risk Calculators A risk factor for Plaquenil hydroxychloroquine retinotoxicity is a daily dose that exceeds 5.0 mg of drug per kg of body weight. The tool on the right simply calculates this threshold based on a ppatient’s real body weight. It’s important to understand that the daily dose is only one risk factor for plaquenil retinotoxicity.
